This program will provide eligible families with supplies of perishable and non-perishable food and drink items on a regular basis. If you are interested, please read the information below carefully to help you fill out the attached form.

  1. Look at the Total Household Income table in the middle of the page. Find the appropriate household size for your family in the first column. Look at the annual, monthly, and weekly household income for your family size. If your family is receiving below those amounts, you are eligible. Circle the ENTIRE LINE that applies to your family.
  2. If you meet the eligibility, please follow steps 3 -5.

If you do not meet the criteria but are still in need of emergency food, please contact Nicole Pollinger at 570-669-9411, option 4.

  1. In the top right corner, print the number of children, adults, and seniors living in the home.
  2. Complete all other items on the form (full name of the recipient, street address, city, state, zip code).
  3. Make sure to sign and date the form and return the school back to the main office.
  4. Please only fill out one form per family. Once we have your completed form on file, you are eligible to pick up food during our distribution times.

You will need to have a completed form on file to pick up food.
